This page is a repository of forms that may be needed for Bai'at (Initiation into the Ahmadiyya Muslim Community), Wasiyyat (Will), Nikah (Marriage), Jalsa (Convention), Registration, etc.

Forms, instructions, and guidance documents provided on this page are for the use of members of Orlando Jama'at. Ahmadis who are not members of the Orlando Jama'at can download and use these documents; however, they are strongly advised to talk to the local Jama'at officials before using any form downloaded from this site.

Please make sure to download any given form every time you need to complete a form. Never use a form previously downloaded and stored on your computer, as forms may have been updated/revised since your last download, Jazakumallah.

Important: Ensure all xxxxx.PDF forms are opened in Adobe Acrobat and not in any browser (Firefox, Chrome, Internet Explorer, etc.). Save the form(s) to your computer by right-clicking on the link and using “Save link as ..” or “Save target as....” Open and complete the form using Adobe Acrobat and then print, sign, and email the completed forms to the Jama'at President.

	Annual Conventions (Jalsa Salana) Forms: Please complete forms at least three weeks before the event, Jazakumallah. None are available at this time.

	Mulaqat (Audience) with Hazrat Khalifatul Masih (may Allah be his Helper): To request Mulaqat with Hazrat Khalifatul Masih (aba), please send a letter requesting mulaqat to Khalifatul Masih at his  address in Islamabad, UK (Please obtain the address from your Jama'at's President)
	Nikah (marriage) Forms and Instructions/Guidelines: Instructions for the Nikah Process Marital Harmony Guide By National Tarbiyyat Department, USA (April 2017) - A Must Read Guidelines for marriage in Islam (Talim & Tarbiyyat Workbook 2015-2017, Lajna Ima'illah, USA) - (for both bride and groom) Introduction to the premarital counseling (PMC) process Premarital Counseling (Required in the US as of July 1, 2016) - Must submit the request at least three weeks before the civil marriage/Nikah. 	Tajnid (Membership): Application for a member number by a new member in the USA Jama'at Change of Address application Addition of a newborn baby Member ID Card (A must for all members) For men For women
	Waqf-e-Nau (New Devotee): Application to obtain Waqf-e-Nau Number Change of country of residence for the Waqf-e-Nau, who has moved to the USA from another country
